It's the difficulty of casting a spell before that initiative in a round.

Specifically, first, you always roll that when fast casting. Every time 
you fast cast. Initiative is rolled only once. Second, an initiative 
roll can't fail. It just indicates the order in which people act. That 
roll can fail, indicating you _can't_ fast cast any more spells 
(including the one you were trying) that round. Third, it says 
explicitly it is not an initiative. That means it is not an initiative 
for purposes of whatever rules deal with initiative, one would assume.

Personally, I think Fast Caster needs updating, which is why I brought 
up the matter. On the other hand, maybe fast caster just applies for 
normal spell casting. That's what the rules indicate, though do not make 
explicit.
